Assessment of the correlation between right ventricular function and pulmonary function in chronic obstructive pulmo-nary disease patients using combined two-dimensional speckle tracking imaging and real-time three-dimensional echo-cardiography

Abstract:Objective This study employed an innovative echocardiographic approach integrating two-dimensional speckle tracking echocardiography(2D-STE)with real-time three-dimensional echocardiography(RT-3DE)to comprehensively evaluate early right ventricular functional alterations in patients with chronic obstructive pulmonary disease(COPD).The primary objec-tives were to characterize patterns of RV structural and functional remodeling and examine their associations with the severity of pulmonary ventilatory impairment.Methods Ninety-one COPD patients were stratified into mild,moderate,and severe groups according to the GOLD 2024 criteria,with 30 healthy individuals serving as controls.RV free wall longitudinal strain(RVFWSL)and global longitudinal strain(RV4CSL)were measured via 2D-STI,while RT-3DE quantified RV ejection fraction(RVEF),end-systolic volume(RVESV),end-diastolic volume(RVEDV),and stroke volume(RVSV).Spearman's rank corre-lation analysis evaluated relationships between RV parameters and pulmonary function indices.Results Compared to the con-trol group,COPD patients across varying severity grades exhibited right ventricular(RV)remodeling,characterized by signifi-cantly decreased RV free wall longitudinal strain(RVFWSL),four-chamber longitudinal strain(RV4CSL),stroke volume(RVSV),and ejection fraction(RVEF),accompanied by elevated end-diastolic volume(RVEDV)and end-systolic volume(RVESV).Spearman's correlation analysis revealed that RVEDV and RVESV were inversely associated with FEV1%(r was-0.476 and-0.436,P<0.05),whereas,the absolute values of RVEF,RVSV,RVFWSL,and RV4CSL demonstrated positive correlations with FEV1%(r was 0.589,0.536,0.599,and 0.429,P<0.05).Conclusion The combined application of 2D-STI and RT-3DE effectively detects early RV structural and functional abnormalities in COPD.Significant RV-pulmonary functional correlations highlight the clinical utility of comprehensive RV quantification for early risk stratification and therapeutic decision-making.
