Diagnostic value of echocardiography for coronary artery lesions and hemodynamic changes in patients with Kawasaki disease

Abstract:Objective To explore the diagnostic value of echocardiography in coronary artery lesions in patients with Kawa-saki disease and to analyze their hemodynamic changes.Methods A total of 110 patients with Kawasaki disease were selected and underwent echocardiography.Hemodynamic indicators such as the mean diastolic velocity(MDV)and peak diastolic veloc-ity(PDV)of the left anterior descending artery,as well as coronary flow velocity reserve(CFVR),were measured.The left coro-nary artery(LCA),right coronary artery(RCA),LCA/aortic root diameter(AO),and RCA/AO were measured to assess the presence of coronary artery lesions.The presence of coronary artery lesions was determined based on subsequent clinical follow-up results.The sensitivity,specificity,accuracy,and consistency of echocardiography in diagnosing coronary artery lesions in Kawasaki disease were analyzed and compared with clinical follow-up results.Hemodynamic indicators were compared between patients with and without coronary artery lesions.Results Among the 110 patients with Kawasaki disease,clinical follow-up di-agnosed 62 cases with coronary artery lesions and 48 cases without.Echocardiography detected 64 cases with coronary artery le-sions and 46 cases without.The sensitivity,specificity,and accuracy of echocardiography in diagnosing coronary artery lesions were 91.94%,85.42%,and 89.09%,respectively,with a high consistency with clinical follow-up diagnoses(Kappa=0.777,P<0.01).Patients with coronary artery lesions in Kawasaki disease had higher MDV and PDV in the left anterior descend-ing artery at rest and lower CFVR(MDV)and CFVR(PDV)compared to patients without coronary artery lesions(P<0.05).Conclusion The patients with coronary artery lesions in Kawasaki disease exhibit coronary artery dilation and increased blood flow velocity.Echocardiography has high diagnostic value for this condition.
