The value of ultrasound combined with serum sICAM-1 and IL-1β in differential diagnosis of granulomatous lobular mastitis and breast cancer

Abstract：Objective This study aims to analyze the value of ultrasound combined with serum soluble intercellular adhesion molecule-1(sICAM-1)and interleukin-1β(IL-1β)in the differential diagnosis of granulomatous lobular mastitis(GLM)and breast cancer.Methods A total of 103 GLM patients were prospectively selected as the lesion group,and 103 breast cancer pa-tients at the same period were regarded as the cancer group,the ultrasound imaging features of the breast were obtained through ultrasound examination.Enzyme linked immunosorbent assay(ELISA)was applied to detect serum sICAM-1 and IL-1β levels.Taking the postoperative pathological results of the enrolled patients as the gold standard,the four grid table was used to analyze the differential diagnostic value of ultrasound in GLM and breast cancer.ROC curve was applied to analyze the differential diag-nostic value of serum sICAM-1 and IL-1β in GLM and breast cancer.The parallel mode was used to calculate the value of ultra-sound combined with serum sICAM-1 and IL-1β in the differential diagnosis of GLM and breast cancer.Results The RI,PSV,and proportion of posterior echo enhancement in the cancer group were obviously higher than those in the lesion group,whereas,the proportion of multiple lesions was obviously lower than that in the lesion group(all P<0.05).The serum levels of sICAM-1 and IL-1β in the cancer group were obviously higher than those in the lesion group(all P<0.05).Among 103 patients with breast cancer,25 were misdiagnosed,and 23 of 103 GLM patients were misdiagnosed as breast cancer.The accuracy,sensitivity and specificity of ultrasound in the differentiation of breast cancer were 76.70%,75.73%and 77.67%,respectively.The area under the curve(AUC)of serum sICAM-1 in the differentiation of breast cancer was 0.838(95%CI:0.784-0.892),the cut-off value was 1 037.010 ng/mL,and both the sensitivity and specificity were 79.61%;the AUC of serum IL-1β in the differentiation of breast cancer was 0.818(95%CI:0.759-0.876),the cut-off value was 7.898 pg/mL,the sensitivity and specificity were 80.58%and 78.64%,respectively.The accuracy of ultrasound combined with serum sICAM-1 and IL-1β in differentiating GLM and breast cancer was 82.04%,and the sensitivity and specificity were 88.35%and 75.73%,respectively.Conclusion Ultrasound combined with serum sICAM-1 and IL-1β can improve the differential sensitivity between GLM and breast cancer,which has cer-tain value.
