Ultrasound combined with strain gauge and shear wave elastography in diagnosis of C-TIRADS 4 thyroid nodules

Abstract:Objective To explore the value of ultrasound combined with elastography technology in diagnosis of C-TIRADS 4 thyroid nodules.Methods A total of 211 C-TIRADS 4 classification thyroid nodules in 186 patients underwent thyroidectomy and proved by the pathological results were included.The hardness was assessed by combined pressure and shear wave imaging.We corrected the thyroid nodules classification,calculated the sensitivity,specificity and accuracy before and after correction,used surgical pathology as the gold standard to draw the ROC curve,and compared the area under the ROC curve before and af-ter correction.Results The sensitivity,specificity and accuracy of the classification for diagnosis of thyroid nodules were 39.87%(63/158),90.57%(48/53),52.61%(111/211).After the correction,these values were 71.88%(115/160),82.35%(42/51),and 74.41%(157/211).The areas under the ROC curve were 0.82 and 0.93 before and after the correction,the difference was statistically significant(Z=2.48,P=0.013).Conclusions The combination of ultrasound with real-time and shear wave elastography is helpful to improve the differential diagnostic accuracy of C-TIRADS4 thyroid nodules and to reduce the excessive diagnosis and treatment of thyroid nodules.
