Characteristics of hemodynamic changes in patients with diabetic retinopathy examined by CDFI combined with FFA and their clinical significance

Abstract:Objective To evaluate the characteristics of hemodynamic changes in patients with diabetic retinopathy(DR)us-ing color Doppler flow imaging(CDFI)combined with fundus fluorescein angiography(FFA)and their clinical significance.Methods 60 patients with DR and 35 patients with simple diabetes were admitted.They were defined as DR group and control group,respectively.All patients were examined with CDFI and FFA.The ultrasonic hemodynamic indicators were compared be-tween the two groups.The value of joint detection of ultrasonic hemodynamic indicators in predicting DR,and the accuracy of FFA in hemodynamic staging diagnosis of patients with DR were analyzed using the ROC curve.The ultrasonic hemodynamic in-dicators in patients with different stages of DR were compared.The value of CDFI combined with FFA in evaluating hemody-namic staging of DR was analyzed using the ROC curve.Results Compared with the control group,DR group had signifi-cantly smaller Vmax and Vmin,and significantly higher RI(all P<0.05).ROC curve analysis results showed that the area under the curve,specificity and sensitivity of the combination of Vmax,Vmin and RI for predicting DR were 0.812,0.756 and 0.724,in-dicating that the combination of ultrasonic hemodynamic indicators was of high value in predicting DR.With the results of fundus angiography as the gold standard,the accuracy of FFA in hemodynamic staging diagnosis of DR was 95.00%(57/60),and Kappa value was 0.857.Patients with proliferative DR had significantly Vmax and Vmin,and significantly higher RI,compared with those with non-proliferative DR(all P<0.05).The area under the curve and specificity of CDFI combined with FFA for predicting hemodynamic staging of DR were 0.843 and 89.5%,which were higher than those of single detection.Conclusion CDFI com-bined with FFA is of high clinical value in evaluating hemodynamic changes and staging of patients with DR.
