Comparative study of enhanced T2-FLAIR sequence and enhanced 3D BRAVO T1WI sequence in AIDS cryptococcal meningoencephalitis

Abstract:Objective To enhance the application of T2-FLAIR sequence and enhanced 3D BRAVO T1WI sequence in the imaging diagnosis of cryptococcal meningitis.Methods 22 patients with AIDS complicated with cryptococcal meningoencepha-litis were retrospectively analyzed.The imaging characteristics of enhanced T2-FLAIR sequence and enhanced 3D BRAVO T1WI sequence were summarized.Then,we calculated the relationship between the location,range,and enhancement mode of lesions between two groups.Results The results showed that the supratentorial lesions of cryptococcal meningitis were mainly charac-terized by enhancement of the pia mater in the frontal,temporal,parietal,and occipital regions,with less enhancement in the pia mater in the bilateral insula regions.Subtopic lesions were more pronounced than supratentorial lesions in terms of uneven en-hancement of the pia mater around the fourth ventricle,brainstem,and bilateral cerebellar hemispheres.When the lesion was ex-tensive,the range displayed between the two groups was basically the same,but when the lesion was mild and accumulated a smaller range of meninges,the enhanced T2-FLAIR sequence was displayed more clearly.There was a statistically significant dif-ference between the two groups in bilateral basal ganglia lesions(gelatinous pseudocysts),all P<0.05.When the lesions accu-mulated in the ventricular membrane of the lateral ventricle,enhanced T2-FLAIR sequence showed more lesions,and there was a statistical difference between the two groups(P<0.05).Conclusion MRI enhanced T2-FLAIR sequence can better display the lesions and scope of cryptococcal meningitis than enhanced 3D BRAVO T1WI sequence,providing important clues for early clinical diagnosis and treatment.
