The value of magnetic resonance imaging in assessing white matter microstructure changes and neurocognitive dysfunc-tion in OSAS patients with obesity

Abstract:Objective To investigate the microstructural changes of magnetic resonance cerebral white matter and neurocog-nitive disorders in patients with obstructive sleep apnea syndrome(OSAS)and obesity.Methods Thirty patients each with non-OSAS and OSAS diagnosed by polysomnography(PSG)were selected.Serum brain-derived neuotrophyic factor(BDNF)de-tection and Montreal Cognitive Assessment(MoCA)were investigated to confirm neurocognitive dysfunction.Routine MRI scan-ning and diffusion tensor imaging(DTI)of the head were performed on all the patients.DTI analysis was conducted on the rel-evance of fractional anisotropy(FA),mean diffusivity(MD),axial diffusivity(AD),radial diffusivity(RD)and all the clinical index.Results Compared with the obese patients in the non-OSAS group,the FA values of some brain regions of the obese pa-tients in the OSAS group decreased(P<0.01),the MD values increased(P<0.05),the AD values decreased(P<0.05),and the RD values increased(P<0.05).The lesions of these patients were mainly located in brain white matter fibers,such as the bilat-eral corona radiata,the bilateral anterior and posterior limbs of the internal capsule,the bilateral cingulate gyrus,and the cor-pus callosum.Correlation analysis showed that FA value was correlated with MoCA,and serum BDNF level was also correlated with MoCA(all P<0.01).Conclusion The cerebral white matter microstructure changes in FA,MD,AD,and RD values were found in OSAS patients.The decrease of FA value and serum BDNF level might be related to neurocognitive dysfunction.
