Clinical value of dynamic contrast-enhanced CT and PET/CT in the diagnosis of mediastinal enlarged lymph node metastasis in patients with lung cancer

Abstract:Objective To explore the clinical value of dynamic contrast-enhanced computed tomography(DCE-CT)and positron emission tomography(PET)/CT in the diagnosis of mediastinal enlarged lymph node metastasis in patients with lung can-cer.Methods A total of 98 patients with lung cancer were selected,and all underwent dynamic contrast-enhanced CT and PET/CT scans.According to presence or absence of metastasis of mediastinal enlarged lymph nodes,patients were divided into metastasis group(67 cases)and non-metastasis group(31 cases).The quantitative parameters of dynamic contrast-enhanced CT,such as CT values of mediastinal lymph nodes in arterial and venous phases,and PET parameters,including maximum stan-dardized uptake value(SUVmax),metabolic tumor volume(MTV),and total lesion glycolysis(TLG)in the two groups,were compared,and the diagnostic value of dynamic contrast-enhanced CT and PET/CT in metastasis of mediastinal enlarged lymph node in patients with lung cancer was analyzed.Results CT values during arterial and venous phases in metastasis group were greater than those in non-metastasis group,and the difference was statistically significant(P<0.05),and SUVmax was higher than that in non-metastasis group,and the difference was statistically significant(P<0.05).There was no significant difference in MTV or TLG between the two groups(P>0.05).The results of ROC curve analysis showed that AUC of CT value in arterial phase combined with CT value in venous phase and SUVmax in the diagnosis of mediastinal enlarged lymph node metastasis was 0.878,greater than that of single index,and the difference was statistically significant(P<0.05).Conclusion Dynamic contrast-enhanced CT combined with PET/CT can improve diagnostic value in mediastinal enlarged lymph node metastasis in patients with lung cancer.
