Clinical efficacy analysis of high resolution CT of the lungs and echocardiography in the diagnosis of chronic obstruc-tive pulmonary disease complicated with pulmonary arterial hypertension

Abstract:Objective To explore the clinical efficacy of high resolution CT of the lungs and echocardiography in the diagno-sis of chronic obstructive pulmonary disease complicated with pulmonary arterial hypertension(PAH).Methods Eighty-six chronic obstructive pulmonary disease(COPD)patients admitted were collected,and all patients underwent high resolution CT of the lungs and echocardiography examinations.The patients were classified into patients with simple COPD(control group,35 patients),those with concomitant mild PAH(PAH in the range of 35~50 mmHg,mild group,29 patients),and those with con-comitant moderately severe PAH(PAH≥51 mmHg,moderately severe group,22 patients)based on the results of echocardio-graphic tests.Results Main pulmonary artery diameter(MPAD),descending aorta diameter(DAD),and ascending aorta di-ameter(AAD)increased sequentially in the control group,mild group,and moderate to severe group(P<0.05).The proportions of residual root sign,right heart shadow enlargement,pleural effusion,and pulmonary exudative changes in the moderate to se-vere group were higher than those in the mild group(P<0.05).There were differences in platelet count,D-dimer,and PaCO2 levels among the control group,mild group,and moderate to severe group(P<0.05).There were differences in forced vital ca-pacity(FVC),forced expiratory volume in one-second(FEV1),and maximal voluntary ventilation(MVV)among the control group,mild group,and moderate to severe group(P<0.05).Conclusion Chest CT has significant diagnostic value for COPD complicated with PAH,which can be accurately determined by measuring MPAD and observing abnormal changes in CT imag-ing.The accuracy of echocardiography in detecting COPD complicated with PAH is high,both of which have good auxiliary ef-fects in diagnosing COPD complicated with PAH.
