Multi parameter quantitative analysis of spectral CT for the diagnosis and analysis of coronary heart disease

Abstract:Objective To explore the diagnostic efficacy of spectral CT multiparametric quantitative analysis for coronary heart disease.Method 123 suspected coronary heart disease patients who underwent spectral CT examination were selected.Based on the diagnostic results of coronary angiography(CAG),the confirmed patients were included in the coronary heart dis-ease group(108 cases),and the others were included in the control group(15 cases).All patients underwent spectral CT exami-nation upon admission,and the spectral CT parameters were calculated as effective atomic number(Zeff),iodine concentration(IC),and standardized iodine concentration(NIC).Receiver operating characteristic(ROC)curve was drawn to analyze the di-agnostic efficacy of spectral CT multiparametric quantification for coronary heart disease.Results The spectral CT examination results showed that Zeff and IC in the coronary heart disease group were lower than those in the control group(P<0.05).Mild ste-nosis patients had higher Zeff,IC,and NIC values than moderate to severe stenosis patients,and moderate stenosis patients had higher Zeff,IC,and NIC values than severe stenosis patients(P<0.05).ROC curve analysis was drawn,with AUC values of 0.775 and 0.855 for Zeff and IC diagnosis of coronary heart disease measured by spectral CT,and optimal cutoff values of 7.754 and 1.980.The AUC values for evaluating moderate coronary artery stenosis using spectral CT measurement of Zeff,IC,and NIC were 0.822,0.853,and 0.755,with optimal cutoff values of 7.425,1.910,and 0.545.The AUC values for evaluating severe ste-nosis were 0.919,0.944,and 0.898,with optimal cutoff values of 6.135,1.585,and 0.465.Conclusion Quantitative analysis of Zeff and IC using spectral CT has diagnostic value for coronary heart disease,and measuring Zeff,IC,and NIC values can evalu-ate the degree of coronary artery stenosis.
