Comparative observation of signal intensity in hepatobiliary phase of gadoxetate disodium(Gd-EOB-DTPA)enhanced MRI for hepatocellular carcinoma patients with different differentiation grades

Abstract:Objective To clarify the relationship between signal intensity in hepatobiliary phase and differentiation grades of hepatocellular carcinoma(HCC)by comparing tumor signal intensity in gadoxetate disodium(Gd-EOB-DTPA)enhanced MRI examinations.Methods We selected 52 pathologically confirmed HCC patients who underwent Gd-EOB-DTPA enhanced MRI within 3 weeks before surgery,including 13 well-differentiated,21 moderately-differentiated,and 18 poorly-differentiated cases.Signal intensities of normal liver parenchyma,tumors,and ipsilateral erector spinae muscle were measured in pre-contrast and hepatobiliary phase sequences.Relative signal intensity(rSI),enhancement rate(ER),and relative enhancement rate(rER)of tumors were calculated and compared among different differentiation groups.Results The tumor rSI values were 47.06%±7.27%,30.95%±8.83%,and 25.53%±4.72%for well-differentiated,moderately-differentiated,and poorly-differentiated HCCs,respectively.Corresponding ER values were 92.03%±6.43%,64.76%±7.01%,and 50.92%±80.62%,while rER values were 754.25%±189.56%,403.82%±158.37%,and 294.78%±67.93%.Well-differentiated HCCs showed significantly higher rSI,ER,and rER than moderately-differentiated and poorly-differentiated groups(all P<0.05).Moderately-differentiated HCCs also demonstrated higher values than poorly-differentiated cases(all P<0.05).Conclusion The signal intensity in hepatobiliary phase of Gd-EOB-DTPA enhanced MRI correlates with HCC differentiation grades,with higher signal intensity,indicating better tu-mor differentiation.
