Magnetic resonance imaging study of plaques in patients with carotid atherosclerosis and chronic obstructive pulmo-nary disease

Abstract:Objective To explore the associations between the severity of chronic obstructive pulmonary disease(COPD)and magnetic resonance imaging(MRI)features of carotid plaque in patients with carotid atherosclerosis(AS)and COPD.Methods All subjects underwent carotid artery MRI.Patients were divided into mild to moderate group(n=51)and severe to ex-tremely severe group(n=46)according to the severity of COPD.Clinical information,carotid morphological parameters and plaque compositions were evaluated.Result A total of 97 patients were included.Compared with mild to moderate group,the incidence of severe lumen stenosis,intraplaque hemorrhage(IPH)and fibrous cap rupture(FCR),as well as the mean wall area,standardized wall index(NWI)and volume of IPH,were significantly increased in severe to extremely severe group(P<0.05).Univariate regression analysis showed that the incidence of severe lumen stenosis,IPH and FCR,as well as volume of IPH were significantly correlated with severe to extremely severe COPD(P<0.05).After adjusting for confounding factors and following multivariate regression analysis,above correlation still had statistical significance.In addition,the inci-dence of lipid-rich necrotic core(LRNC)and NWI were also significantly correlated with severe to extremely severe COPD.Conclusion High plaque burden and vulnerable plaque components are both associated with the severity of COPD.
