Comparison of CTP parameters and CTA scores with different prognosis and hemorrhagic transformation in acute ischemic stroke

Abstract:Objective To explore the relationship between total cerebral perfusion(CTP)parameters and CT angiography(CTA)score with the prognosis and hemorrhagic transformation in acute ischemic stroke(AIS).Methods 64 patients with AIS underwent CTP,CTA examination,CTP parameters(Core infarct volume,rCBV,rCBF,HIR,Tmax)were measured,then,CTA images were reconstructed and the status of collateral circulation was evaluated according to the CTA scoring criteria.The correlation between CTP parameters and CTA score was analyzed.Hemorrhagic transformation was determined according to the follow-up imaging 24 hours later,and the differences in CTA score and CTP parameters(core infarct volume,rCBV,rCBF,rPS,rMTT,HIR,Tmax)of patients with(22 cases)and without(42 cases)hemorrhagic transformation were analyzed.We ob-tained the modified Rankin Scale(mRS)score of the patient 3 months later,and analyzed the differences in CTA scores and CTP parameters(core infarct volume,HIR,rCBV)for patients with good prognosis(41 cases)and poor prognosis(23 cases).Results The CTA score was positively correlated with rCBV(P<0.01),and negatively correlated with HIR and core infarct volume(all P<0.05).Compared with the non-hemorrhagic patients,the HIR and rPS were higher in the hemorrhagic transformation patients,while,rCBF,CTA score and rCBV were lower(P<0.05).Compared with good prognosis patients,CTA scores,rCBV were lower in the poor prognosis patients,while,HIR,core infarct volume were higher(P<0.05).Conclusion CTP parameters and CTA scores are correlated in AIS patients.HIR and rPS are higher and CTA scores and rCBV are lower in AIS pa-tients with hemorrhagic transformation and poor prognosis.
