The predictive value of T1-mapping and T2-mapping techniques for myocardial hypoperfusion after PCI in acute myo-cardial infarction

Abstract:Objective To investigate the value of T1-mapping and T2-mapping techniques in predicting myocardial hypoperfu-sion after percutaneous coronary intervention(PCI)for acute myocardial infarction(AMI).Methods A total of 163 patients with AMI were selected,all of whom underwent PCI,and were divided into myocardial hypoperfusion group(n=36)and normal group(n=127)according to postoperative TIMI results.The T1-mapping parameters in terms of initial T1 value,enhanced T1 value and extracellular volume fraction(ECV),T2-mapping parameters(T2 value),and their predictive value were compared between the two groups.Results Compared with normal group,the initial T1,ECV and T2 values in hypoperfusion group were higher,and the T1 values after enhancement were lower(P<0.05).Myocardial hypoperfusion after PCI was positively correlated with initial T1,ECV and T2 values,and negatively correlated with enhanced T1 values(P<0.05).The AUC of the combined pre-diction of myocardial hypoperfusion after PCI was 0.937,the specificity was 81.10%,and the sensitivity was 94.44%,which was significantly better than that of single prediction.The incidence of myocardial hypoperfusion in patients with high initial T1 values,ECV values,and T2 values was 2.506,2.086,and 3.543 times of that in patients with low initial T1 values,and 0.470 times of that in patients with high initial T1 values after enhancement(P<0.05).Conclusion Myocardial hypoperfusion in AMI patients after PCI is closely related to the relevant parameters of T1-mapping and T2-mapping,and the combined detection has high predictive efficacy,which is helpful to guide clinical diagnosis and treatment and to improve prognosis.
