Study on the evaluation of collateral circulation in acute cerebral infarction using CT perfusion imaging combined with angiography

Abstract:Objective To explore the clinical value of CT perfusion imaging(CTP)combined with angiography(CTA)in evaluating collateral circulation in acute cerebral infarction(ACI).Methods 128 patients with acute cerebral infarction(ACI)were selected as the research subjects.All patients underwent CTP,CTA,and digital subtraction angiography(DSA)examina-tions after admission.Based on the DSA examination results,the patients were divided into a good collateral circulation group and a poor collateral circulation group.The CTP and CTA examination results of the two groups were compared,and the evalua-tion value of CTP combined with CTA for ACI collateral circulation was explored through the receiver operating characteristic curve(ROC).Results Among 128 ACI patients,78 cases were in the good collateral circulation group and 50 cases were in the poor collateral circulation group.The average transit time(MTT)and peak time(TTP)of the poor collateral circulation group were longer than those of the good collateral circulation group.The regional leptomeningeal collateral circulation score(rLMC score)and luminal area(LA)were smaller than those of the good collateral circulation group(P<0.05).ROC curves were drawn,indicating that the area under the curve(AUC)values for MTT,TTP,rLMC scores,and LA alone and in combination for assessing collateral circulation in ACI patients were 0.626,0.748,0.705,0.764,and 0.893,respectively,with the highest value for combined assessment.Conclusion CTP can evaluate cerebral hemodynamics,CTA can evaluate collateral circula-tion status and luminal area,and CTP combined with CTA can improve the evaluation value of collateral circulation in ACI.
