Clinical imaging and prognostic features of children with first episode of MOG-IgG-positive myelitis

Abstract:Objective To investigate the clinical,radiological characteristics and prognostic status of children with a first episode of anti-myelin oligodendrocyte glycoprotein immunoglobulin G(MOG-IgG)-positive myelitis.Methods This was a ret-rospective study.The clinical and imaging data of MOG-IgG positive children(≤18 years old),who were hospitalized in our neu-rology department with a first episode of central nervous system demyelinating event and symptoms of myelitis,were collected.The clinical,imaging characteristics,and prognosis were analyzed.Results 22 children were included finally.The most com-mon symptom was limb weakness(90.9%),followed by sensory abnormalities(50.0%)and urinary and bowel dysfunction(50.0%).Magnetic resonance imaging features included solitary(11 cases)or multiple lesions(11 cases),long segmental(16 cases)or short segmental lesions(11 cases),and prominently involved gray matter.Linear signs and"H"signs were seen in 12 cases,and conus involvement occurred in 9 cases.Spinal cord patchy enhancement was observed in 3 of 11 cases.By the last follow-up(ranged from 0.6 to 4.8 years,with a median time of 2.4 years),3 children were observed to have relapses,19 patients had complete clinical recovery,and 3 patients retained mild neurological deficits.Spinal cord atrophy in subsequent MRI follow-up showed a correlation with EDSS scores(r=0.998,P<0.001).Conclusions The imaging of the first onset of MOG-IgG posi-tive myelitis in children is mostly characterized by long segment lesions with central gray matter involvement,and can also be manifested as short segment lesion type myelitis,which may be solitary or multiple.Involvement of the conus is common.Most children have good prognosis and low recurrence rate.MRI evidence of spinal cord atrophy may indicate a poor clinical prognosis.
