CT and MRI features of rare splenic tumors and literature review

Abstract:Objective To investigate the CT and MRI imaging features of rare splenic tumors,along with review of litera-ture.Methods The CT and MRI features of 23 patients with rare splenic tumors,who all underwent CT and MR examination,confirmed by operation and pathology,were compared and analyzed,retrospectively.Results Primary lymphoma in 13 cases appeared as slightly low-density on CT,iso-hypointensity on T1WI and isointensity or hyperintensity on DWI and T2WI,with het-erogeneously mild to moderate enhancement on dynamic contrast enhancement,and 2 showed vascular floating sign.Primary an-giosarcoma of the spleen in 4 cases accompanied by extrasplenic metastasis at the time of diagnosis.CT showed enlargement of the spleen with cystic and hemorrhage,heterogeneous intensity on T1WI and T2WI,hyperintensity on DWI,and progressively ob-vious enhancement.Isolated splenic metastasis in 3 cases appeared as mixed density with moderate enhancement in solid part,while,cystic part manifested no enhancement.Inflammatory myofibroblastic tumor in 2 cases showed low to moderate density on CT,heterogeneously hypointense on T1WI,T2WI and DWI,isointense intratumoral intervals were observed,with"slow in,slow out"heterogeneous enhancement pattern.One patient with EBV+inflammatory follicular dendritic cell sarcoma showed large mass with cystic and calcification on CT,isointensity on T1WI,T2WI and mild hyperintensity on DWI,which were progressively isointense on delayed phase with"fast in,slow out"pattern.Conclusion The imaging features of rare tumors in the spleen have certain characteristics,and the combination of extrasplenic manifestations and clinical features can provide references for diagnosis.
