The value of 18F-FDG PET/CT and tumor markers in diagnosis and differential diagnosis of pancreatic adenosquamous carcinoma

Abstract:Objectives To investigate the characteristics of 18F-fluorodeoxy glucose(FDG)positron emission tomography/computed tomography(PET/CT),contrast-enhanced CT and tumor markers of pancreatic adenosquamous carcinoma(PASC),and to assess their values in the diagnosis of PASC and pancreatic ductal adenocarcinoma(PDAC).Methods 7 patients with PASC and 20 patients with PDAC were included in this retrospective study.The clinical information,PET/CT image features,contrast-enhanced CT image features,and tumor markers were analyzed.t-test,Mann-Whitney U test,and Fisher exact test were used to compare the differences according to the type of data.The value of different indicators in the diagnosis of PASC was analyzed using receiver operating characteristic(ROC)curves to obtain the area under the ROC curve(AUC).Results The differences in the maximum diameter(t=-2.864,P=0.008),clear margin(P=0.001),exophytic growth(P=0.006),distal pan-creatic atrophy(P=0.026),SUVmax(Z=-3.819,P<0.001),ring-like 18F-FDG metabolism(P=0.002),ring-like enhancement(P=0.023),and SCCA(Z=-3.375,P=0.001)value between the two groups were statistically significant.When differentiated PASC from PDAC,the AUC and specificity of the SUVmax were higher than parameters(0.993;95.0%);the sensitivity of both the SUVmax and the distal pancreatic atrophy were higher than parameters(100.0%).Conclusions The central necrosis,ring-like 18F-FDG metabolism pattern,SUVmax,and SCCA have good predictive values in the differential diagnosis of PASC and PDAC.Among the parameters,SUVmax has the best diagnostic efficacy.
