Study on the diagnostic efficacy of ADC value in the darkest area of magnetic resonance ADC map for benign and ma-lignant breast lesions

Abstract:Objective To explore the diagnostic efficacy of different apparent diffusion coefficient(ADC)values in the darkest area of breast magnetic resonance imaging(MRI)ADC maps for differentiating benign and malignant lesions.Methods Breast MRI data of 275 patients(286 lesions in total)confirmed by surgical pathology or biopsy were collected.The region of interest(ROI)was selected in the darkest area of the lesion ADC map,and the minimum ADC(ADCmin),mean ADC(ADCmean),and maximum ADC(ADCmax)were measured.According to the pathological results,the lesions were divided into a benign group(120 lesions)and a malignant group(166 lesions).The area under the receiver operating characteristic(ROC)curve(AUC)was used to evaluate the diagnostic performance of each ADC value.The diagnostic efficacy of different ADC val-ues for benign and malignant lesions was compared by DeLong test.Results The AUCs of ADCmin,ADCmean,and ADCmax for differentiating benign and malignant lesions were 0.938,0.935,0.911,respectively.DeLong test showed that there was no statisti-cal difference between ADCmin and ADCmean(Z=0.428,P=0.672);there were statistically significant differences between ADC-min and ADCmax,and between ADCmean and ADCmax(Z=2.352,P=0.020;Z=3.647,P<0.001).For small lesions with a maxi-mum diameter of≤1 cm,there was no statistically significant difference in diagnostic performance between ADCmax,ADCmin,and ADCmean in pairwise comparisons(P>0.05).Conclusion ADCmin and ADCmean in the darkest area of the magnetic reso-nance ADC map have high diagnostic performance for differentiating benign and malignant breast lesions.
