Value of spectral CTA combined with clinical data to predict unfavorable penumbra pattern in stroke

Abstract:Objective To explore the value of quantitative parameters of spectral CT brain CTA combined with clinical data in predicting unfavorable penumbra pattern in patients with acute ischemic stroke.Methods The clinical and imaging data of 53 patients evaluated as acute ischemic stroke in Weihai Central Hospital were collected retrospectively.With CTP assessment result(matching of infarction and ischemic area)as the reference standard,they were divided into favorable and unfavorable penumbra pattern groups.Binary logistic regression was applied to explore predictors of unfavorable penumbra pattern,and fur-ther,the nomogram model was conducted.ROC curve was used to evaluate degree of discrimination,calibration curve and deci-sion curve were used to evaluate accuracy and clinical utility of the model.Results Binary logistic regression showed that io-dine ratio,collateral circulation score and baseline National Institutes of Health Stroke Scale(NIHSS)score were influencing factors(all P<0.05).The ROC area under curve was 0.885(95%CI:0.778-0.993);the calibration curve and the ideal curve fit-ted well;the decision curve analysis showed that when the high risk threshold was 12%-100%,the standardized net benefit of pa-tients was greater than zero.Conclusions The nomogram model constructed based on spectral CTA quantitative parameters and clinical data has good prediction efficacy,which can accurately predict unfavorable penumbra pattern and provide an impor-tant reference for clinical treatment.
