The diagnostic value of MRI in non-small cell lung cancer with leptomeningeal metastasis involving cranial nerve

Abstract:Objective We purposed to evaluate the clinical efficacy of magnetic resonance imaging(MRI)in diagnosing lep-tomeningeal metastasis(LM)in non-small cell lung cancer(NSCLC)patients on non-contrast-enhanced(NCE)and contrast-enhanced(CE)3D-T2 FLAIR,CE 3D-T1WI,and CE 3D-T1 MATRIX sequences,aiming to assess the diagnostic value of these MRI in identifying LM involvement of the cranial nerves.Methods 133 patients of NSCLC with LM in our hospital were en-rolled in the prospective study.MRI data from these patients were evaluated using NCE/CE 3D-T2 FLAIR,CE 3D-T1WI and CE 3D-T1 MATRIX.The imaging characteristics of cranial nerve involvement in LM were specifically analyzed.Results In NSCLC patients with LM,80.45%(107/133)had cranial nerve metastasis,the cranial nerves Ⅱ,Ⅲ,Ⅴ and Ⅶ~Ⅷ were the most com-monly involved,followed by the cranial nerve Ⅸ.Cranial nerve metastasis often presented as enlargement of the nerve with linear enhancement,and cranial nerves were usually bilaterally affected,and often multiple groups of cranial nerves showed abnor-malities simultaneously.The consistency of the imaging feature evaluation by two radiologists was 0.926.Conclusion When symptoms indicative of cranial nerve abnormalities arise in NSCLC patients with LM,it is essential to meticulously analyze the clinical manifestations and imaging features.This approach aims to enhance the MRI diagnostic accuracy for cranial nerve in-volvement in LM.
