Application of chest CT in grading diagnosis of AECOPD combined with pulmonary hypertension

Abstract：Objective To investigate the application value of chest CT in the grading diagnosis of pulmonary hypertension(PH)in acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods 80 patients with AECOPD com-bined with PH admitted to Guangde Traditional Chinese Medicine Hospital were selected as the study subjects.Based on the pul-monary artery systolic pressure(PASP)values detected by echocardiography,the patients were divided into mild group(PASP:35-50 mmHg)of 48 cases,moderate group(PASP:51-70 mmHg)of 23 cases,and severe group(PASP:>70 mmHg)of 9 cases.The chest CT examination parameters of patients with different grades were compared,and the value of chest CT in the grading diagnosis of AECOPD combined with PH was explored through receiver operating characteristic curve(ROC).Results Among 80 patients with AECOPD complicated with PH,48 cases had mild PH,and mild increases in pulmonary artery diam-eter,ascending aorta diameter,and descending aorta diameter were observed;23 cases of moderate PH showed significant in-creases in the diameter of the main pulmonary artery,ascending aorta,and descending aorta.Nine cases of severe PH showed significant increases in the diameter of the main pulmonary artery,ascending aorta,and descending aorta.Comparing the param-eters of main pulmonary artery diameter(MPAD),ratio of main pulmonary artery to ascending aorta diameter(rPA),and right pulmonary artery diameter(RPAD)among the three groups,the mild group was less than the moderate group and less than the severe group(P<0.05).Comparing the diameter ratio of the main pulmonary artery to the descending aorta(rPD)and the diam-eter of the left pulmonary artery(LPAD)among three groups,there was no statistical significant difference(P>0.05).ROC curve was drawn to show that the AUC values of chest CT examination parameters alone and in combination for diagnosing AECOPD combined with PH grading were 0.795,0.733,0.679,and 0.904,respectively,indicating that the combined diagnos-tic value was the highest.Conclusion Chest CT has good application value in the grading diagnosis of AECOPD combined with PH,which can provide valuable reference for the development of clinical diagnosis and treatment plans for AECOPD combined with PH.
