Clinical analysis of CT plain scan combined with signs in predicting early hematoma enlargement in spontaneous cere-bral hemorrhage

Abstract：Objective To analyze the value of CT plain scan combined with signs in predicting early hematoma enlargement in spontaneous intracerebral hemorrhage(sICH).Methods The clinical data of 86 patients with sICH who were treated in the hospital were selected by non-random sampling method.According to the CT scan results within 24 hours,they were divided into a hematoma enlargement group(n=36)and a non-enlargement group(n=50).The general data and CT plain scan imaging data of the two groups were compared.Logistic regression was used to explore the factors of early hematoma enlargement in patients with sICH,and the value of CT plain scan combined with signs in predicting early hematoma enlargement in patients with sICH was analyzed.Results In CT plain scan,in the hematoma enlargement group,there were 4 cases with only 1 sign,4 cases with 2 signs,18 cases with 3 signs,8 cases with 4 signs,and 2 cases with 5 signs.In the non-enlargement group,there were 16 cases with only one sign,17 cases with two signs,10 cases with three signs,and 7 cases without signs.The initial hematoma vol-ume in the hematoma enlargement group was larger than that in the non-enlargement group,and the proportions of mixed sign,black hole sign,vortex sign,island sign,and combined signs were all higher than those in the non-enlargement group(P<0.05).Logistic regression showed that initial hematoma volume(OR=1.310),mixed sign(OR=4.031),whirlpool sign(OR=4.411),is-land sign(OR=4.753),and combined sign(OR=4.967)were all risk factors for early hematoma enlargement in patients with sICH(P<0.05).Receiver operating characteristic(ROC)curves was drawn,the area under the curve for predicting early hema-toma enlargement in sICH using mixed sign,vortex sign,island sign,and combined sign was 0.668,0.681,0.709,and 0.761,respectively.The value of combined sign prediction was relatively high.Conclusion The combination of CT plain scan and signs is associated with the risk of early hematoma enlargement in sICH.The combination of signs can indicate the risk of early hematoma enlargement,which has certain predictive value.
