Diagnostic value of head-neck vessel wall imaging technique based on compressed sensing in ischemic cerebrovascular disease

Abstract:Objective To explore the relationship between atherosclerotic plaque characteristics and blood perfusion in the head and neck region of patients by applying head-neck vessel wall imagingtechnique(HNJ-VWI).Methods 103 patients with atherosclerosis were selected for inclusion in the study.The correlation between plaque distribution and stroke type was ana-lyzed.Twenty-two patients with single plaques in the middle cerebral artery were divided into low-perfusion(n=77)and normal-perfusion(n=26)groups according to the MTT-Alberta Stroke Program Early CT Score(MTT-ASPECTS),and plaque character-istics were analyzed.Logistic regression analysis and ROC curves were used to analyze the effect of plaque characteristics on hy-poperfusion.Results The number of plaques in the low perfusion group were more than those in the normal perfusion group,and the difference between the two groups was statistically significant(P<0.05),and in patients with atherosclerotic stroke,the rate of recurrent infarction in the intracranial-extracranial coexistence group was higher than that in the intracranial group(42.55%versus 14.81%in coexistence versus the intracranial,respectively,P<0.05).Comparing the middle cerebral artery single plaque low-perfusion group and normal-perfusion group,the differences in plaque enhancement grade,length,and vessel stenosis between the two groups were statistically different(P<0.05);plaque enhancement grade,vessel stenosis,and plaque length were moderately correlated with MTT-Aspects(P<0.05).Plaque length[P=0.045,OR=1.722(95%CI:1.012-2.928)]was independently associated with hypoperfusion.Conclusion Combining the degree of plaque enhancement,vessel stenosis,and plaque length improves discrimination between low-and normal-perfused patients.HNJ-VWI is important for evaluating ath-erosclerotic plaques throughout the head and neck.
