The findings and clinical value of contrast-enhancement MRI,diffusion-weighted imaging and apparent diffusion coeffi-cient in hepatocellular carcinoma with portal vein tumor thrombus

Abstract:Objective To explore the findings and clinical value of contrast-enhancement MRI,diffusion-weighted imaging(DWI)and apparent diffusion coefficient(ADC)in primary hepatocellular carcinoma with portal vein tumor thrombus(PVTT).Methods A retrospective analysis was conducted on the clinical data of 38 patients with HCC and PVTT.All patients under-went contrast-enhancement MRI,DWI and ADC scan before surgery or interventional therapy.The imaging manifestations of contrast-enhancement MRI,ADC and DWI between the HCC lesions and PVTT were compared.Then,we measured the ADC values and analyzed its relevance between the HCC lesions and PVTT.Results Of the 38 patients,49 HCC lesions and 75 PVTT were found by MRI.45 HCC lesions were significantly enhanced in arterial phase,DWI image showed high signal and ADC image showed low signal in 46 and 45.72 PVTT were significantly enhanced in arterial phase,DWI image showed high sig-nal and ADC image showed low signal in 71 and 72.The imaging manifestations of enhanced on the arterial phase,high signal on DWI and low signal on ADC between the HCC lesions and PVTT were shown no significant difference(P>0.05).The average ADC value of HCC lesions and PVTT were(1.25±0.16)×10-3mm2/s and(1.08±0.10)×10-3mm2/s,the difference was statistically significant(P<0.05),and there was a correlation of ADC values between HCC lesions and PVTT(r=0.381,P<0.05).Conclu-sion The imaging findings,such as arterial phase enhancement,DWI high signal,ADC low signal and low ADC value of PVTT,have a high clinical value for its accurate diagnoses.
