Evaluation and analysis of head CT combined with MRI-DWI in acute cerebral infarction

Abstract:Objective To explore the evaluation value of head CT combined with MRI-DWI for acute cerebral infarction.Methods A prospective study was conducted on 150 suspected acute cerebral infarction patients with symptoms consistent with acute cerebral infarction.All patients underwent digital subtraction angiography(DSA),cranial computed tomography(CT),and magnetic resonance imaging(MRI)diffusion-weighted imaging(DWI).DSA was used as the gold standard,and patients were divided into infarction group and other disease group based on the diagnostic results of the gold standard.The imaging pa-rameters of cranial CT and MRI-DWI were compared between the two groups to analyze their evaluation efficacy for acute cere-bral infarction.Results Among 150 patients with suspected acute cerebral infarction,128 cases were diagnosed by DSA,ac-counting for 85.33%.Compared with other disease groups,the CT value,ADC value and DCavg value of the infarction group were lower(P<0.05).Receiver operating characteristic(ROC)curves were drawn,and the results showed that CT values and ADC values had certain efficacy in evaluating acute cerebral infarction,and the combined evaluation had higher efficacy.DSA was used as the gold standard,the optimal threshold for evaluating acute cerebral infarction was obtained through ROC.Based on the threshold judgment,the evaluation results showed that the detection rate of acute cerebral infarction in head CT examination was 79.69%(102/128),the detection rate of MRI-DWI was 86.72%(111/128),and the combined detection rate was 94.53%(121/128).The combined detection rate was higher,and the sensitivity of the combined examination was higher than that of the individual examination.Its accuracy and negative predictive value were higher than those of head CT examination(P<0.05).The consistency between the combined examination and the gold standard was stronger.Conclusion The combination of head CT and MRI-DWI can effectively evaluate acute cerebral infarction,and has good consistency with the gold standard,which has cer-tain application value.
