MRI diagnosis of DWI hyperintensity lesions in cerebral cortex

Abstract:Objective To investigate the value of MRI in the diagnosis of DWI hyperintensity lesions in Cerebral Cortex. Methods We retrospectively analyzed of 32 cases of DWI hyperintense of Cerebral Cortex. Results The lesions included 10 Creutzfeldt Jakob disease ( CJD) , 10 cases had no swelling of cerebral cortex and the imaging characteristics of CJD on DWI were symmetrical or asymmetrical findings of high signals on basal ganglia and cerebral cortex, except globus pallidus. 3 mitochondrial encephalomyopathy ( ME) mainly involved the posterior cortex of the brain ,and showed slightly high signal intensity on DWI. 4 hypoxic-ischemic encephalopathy ( HIE) were that the lesions of cortex were consistent with vascular distribution, 2 cases had swelling of cerebral cortex with slightly hyperintensity on DWI. In 11 encephalitis, 4 cases of cerebral cortex were swelling, and the DWI high signal changed earlier than T2 WI. 2 extrapontine myelinolysis ( EPM) lesions were mainly distributed at the junc-tion, and manifested little high signal on DWI. 1 hypoglycemic encephalopathy ( HE) , and the characteristics were the affected areas of the parietal and occipital lobes and high signal on DWI. 1 reversible posterior encephalopathy syndrome ( RPES) was u-sually located in parieto-occipital lobes,especially in white matter, but they could also be seen in gray matter. Conclusion Diag-nosis and differential diagnosis of DWI hyperintensity lesions in cerebral cortex were combined with the location of the lesions and the characteristics of MRI.
