The value of Siemens 1.5 T dynamic contrast-enhanced magnetic resonance combined with diffusion weighted imaging in evaluating clinical efficacy of neoadjuvant chemotherapy in breast cancer

Abstract:Objective To investigate the value of Siemens 1.5 T dynamic contrast-enhanced magnetic resonance combined with diffusion weighted imaging in evaluating clinical efficacy of neoadjuvant chemotherapy in breast cancer. Methods The clini-cal data of 32 cases with breast cancer by pathological biopsy diagnosis treated by NAC before operation in our hospital from April 2015 to November 2016 year were collected. The longest and shortest diameter examined by MRI and B-ultrasound examination were compared. The main characteristics of cancer foci on the MRI images were analyzed. The value of DCE-MRI combined with DWI examination in judging the axillary lymph node metastasis were investigated. Results In 32 patients, before neoadjuvant chemotherapy,MRI images with multiple nodules or masses were common,after neoadjuvant chemotherapy,21.87%(7/32) im-age showed the appearance of irregular shape, the boundary was still clear, presenting with patchy, image features for the en-hancement of the delayed phase. 78.12%(25/32) patients for image displayed solitary nodule, formed a more rules, clear boundary,image features mainly reduced to concentric;The r correlation coefficient between the longest and shortest diameter by MRI scan and the longest and shortest diameter by pathological examination were 0.851 and 0.671( P <0.001),which was sig-nificantly higher than that of B-ultrasound( r =0.634,0.612,P <0.01). The sensitivity and specificity of MRI scan in judging whether axillary lymph node was metastasis compared with the results of pathological examination were 45.83%(11/24) and 50.00%(4/8),and the sensitivity and specificity of B ultrasound scanning in judging whether axillary lymph node was metastasis were 29.16%(7/24) and 12.50%(1/8) respectively. The sensitivity and specificity of MRI scan in the diagnosis of axillary lymph node metastasis were significantly better than those of B ultrasonography ( P <0.05). Conclusion 1.5 T dynamic con-trast-enhanced magnetic resonance combined with diffusion weighted imaging can accurately evaluate the scope and appearance of focus and whether axillary lymph node was metastasis after NAC,which provide reliable information for clinical treatment.
