CT angiography manifestation and comprehensive interventional therapy in patients with acute pulmonary embolism

Abstract:Objective To analyze the 64-slice spiral CT angiography (CTA) manifestation and curative effect of comprehen-sive interventional therapy in patients with acute pulmonary embolism(PE). Methods Data of 57 patients with acute PE treated in our institution from Jun 2014 to Jan 2017 were collected. Direct and indirect CTA signs were analyzed retrospectively. All pa-tients'dignoses were confirmed by pulmonary digital subtraction angiography and then were performed with comprehensive therapies including placement of inferior vena cava filter,catheter thrombolysis of pulmonary artery,breaking thrombus,sucking thrombus with negative pressure. Effect evaluations were performed using clinical symptoms and signs,vascular recanalization,oxygen par-tial pressure,oxyhemoglobin saturation,pulmonary artery pressure,and Miller score. Results Totally,57 PE patients were di-agnosed by CTA. And size,location,amount,and embolization degree of thrombus were clearly shown by CTA. The CTA direct sign of PE was filling defect,and the indirect sign included thin lung marking,expanding pulmonary artery,pulmonary infarction and hydrothorax. The success rate of operation was 93%, and all manifestations, such as dyspnea, chest distress, pectoralgia, cough,hemoptysis,were improved. The pulmonary artery pressure,oxyhemoglobin saturation,oxygen partial pressure,and Mill-er score were also improved (all P<0.01). The other 4 PE patients died due to severe illness. Conclusion The diagnosis and assessment of PE can well be done by 64-slice spiral CTA. The interventional treatment for acute PE is safe and effective,reduc-ing ameliorative symptoms and degressive mortality.
