Radiological and pathological features of desmoplastic infantile tumors

Abstract:Objective To analyze and summarize the radiological and pathological features of desmoplastic infantile tumors (DITs),and to improve the diagnosis ability of the disease.Methods We retrospectively collected and analyzed 6 cases of DITs,which were confirmed by pathology after resection.Preoperative head CT scan was performed in 3 patients and MRI in 5 cases.Results The 6 cases included 2 females and 4 males,and aged from 2 months to 19 months.All the 6 DITs occurred in cerebral hemisphere,and were single and massive and predominandy cystic with a solid component.The radiological findings of the cystic components were close to that of the cerebrospinal fluid.The solid portions were peripheral with leptomeningeal contact,which were heterogeneous,mainly had slight high density and isointense and no evidence of restricted diffusion.The solid components of tumors showed intense enhancement with adjacent leptomeninges enhancement in 6 cases and dural adhesion in 3 cases.Intraoperatively,it was seen that cystic fluid was light yellow,the solid components were gray white and tough with moderate blood supply.Pathologic examination showed desmoplasia,astrocytic tumor cell in hyperplasia fibrous stroma,and ganglial tumor cells in 4 cases.Conclusion DITs occur predominantly in two years of age.The main imaging features of DITs include large tumors locate in cerebral hemisphere with both solid and cystic components;the solid portions are smaller than cystic portions,which are peripheral with meningeal contact,are mainly isointense to gray matter,and have no restricted diffusion and have intense enhancement.
