The value of enhanced contrast ratio of magnetic resonance imaging (MRI) in the diagnosis of benign and malignant lesions of breast

Abstract:Objective To investigate the value of enhanced contrast ratio of magnetic resonance imaging (MRI) in the diagnosis of benign and malignant lesions of breast.Methods A retrospective analysis was performed on 59 patients with pathologically confirmed and dynamic contrast-enhanced MRI of the breast before operation.There were 32 malignant lesions and 27 benign lesions.ROI, the peak time, time signal intensity curve (TIC), early enhancement ratio, early enhancement rate were recorded, the number of blood vessels and vascular diameter within 4 cm of the lesion were recorded.We used rank sum test to compare the difference between benign and malignant breast lesions of MRI parameters and used ROI curve to evaluate the pathological changes of blood vessel diameter, early enhancement rate, early enhancement ratio, early enhancement ratio combined with vascular diameter in differentiating benign and malignant breast lesions.Results The median of early breast malignant lesions enhancement rate, peak time, early enhancement ratio, the number of blood vessels and vascular diameters were 173%, 2.1 s, 100%, 5, 2.98 mm, the above parameters in benign lesions were 112.3%, 4.8, 81.2%, 0 s, 0 mm, the differences were statistically significant (all P <0.05).The sensitivity and specificity were 93.19% and 67.85% with a threshold of 85.3%, respectively, which were identified by the early enhancement ratio of ROC in benign and malignant breast lesions;The area under the ROC of the early enhancement rate was 0.721, and the sensitivity and specificity of identification with the threshold of 119% were about 81.73% and 55.15%, respectively;The area under the ROC of the vessel diameter was 0.872, and the sensitivity and specificity were 73.61% and 85.14%, respectively, at the threshold of 2.79 mm;The area of ROC was 0.919 in the diagnosis of early enhancement ratio combined with peripheral vascular diameter.The diagnostic sensitivity and specificity were 96.3% and 63.40%, respectively.Conclusion Dynamic contrast-enhanced MRI parameters, the early enhancement ratio and the diameter of the vessels around the lesions are of high sensitivity in differentiating benign and malignant breast lesions, which has good clinical application value.
