Ultrasonic evaluation on the intracranial stenosis or occlusion of internal carotid artery via unilateral extracranial examination
Abstract:Objective To explore the clinical application value of ultrasound examination on extracranial internal carotid artery in the assessment of intracranial stenosis or occlusion of ipsilateral internal carotid artery.Methods We retrospectively collected the systolic blood flow peak velocity of extracranial internal carotid artery or RI>0.75 in 78 patients with atherosclerosis.Then, we used ROC curve, selected the meaningful parameters and cutoff value (RI=0.885), and divided those patients into two groups for statistical analysis according to the cutoff value.Results Of those 78 patients with atherosclerosis, 40 had the intracranial stenosis of ipsilateral internal carotid artery, 38 had no intracranial segment period stenosis of ipsilateral internal carotid artery, 54 set up intracranial collateral circulation, and 24 had no collateral circulation.Taking RI=0.885 as cutoff value, the diagnostic accuracy was 88.3%, sensitivity was 93.9%, specificity was 88.3%, positive predictive value was 77.5%, and negative predictive value was 94.7%, with statistical significance.Conclusion The ultrasound examination on extracranial internal carotid artery has clinical significance in the assessment of intracranial stenosis or occlusion of ipsilateral internal carotid artery.
