Application value of quantitative analysis of DWI in the differential diagnosis of Graves disease hyperthyroidism and early Hashimoto's thyroiditis

Abstract:Objective To study application value of ADC values of different b values in thyroid DWI imaging in identification of Graves disease hyperthyroidism and early Hashimoto thyroiditis.Methods We collected 12 patients with Graves disease hyperthyroidism, who were made a definite diagnosis, 10 cases of early Hashimoto's thyroiditis and 10 cases of healthy persons as a control group.Axial DWI scans of thyroid with b=150, 300, 550 were performed respectively and apparent diffusion coefficient (ADC) values of thyroid bilateral leafs were measured, then, the difference of ADC value of different groups with the single factor analysis of variance was detected.When the b values was the same, P<0.05 was statistically significant.We selected the threshold of Hashimoto's thyroiditis group of different b value with the ROC curve, and then, calculated Sensitivity and specificity.Results As the b values increased, ADC values decreased in the same group;The ADC values were significantly different in the different groups with the same b values (P<0.05).In the group of early Hashimoto's thyroiditis, when b=150, 300, 550 and respectively ADC <1.87×-3 mm2/s, <1.66×10-3 mm2/s, <1.26×10-3 mm2/s, the sensitivity was 100%, specificity was higher than 85%.Conclusion DWI quantitative analysis of different b values for the sensitivity and specificity of early Hashimoto's thyroiditis are very good.Especilally, when b=550, ADC <1.66×10-3 mm2/s, the application value is more prominent.Quantitative analysis of DWI is important significantly in the differential diagnosis of Graves diseasehyperthyroidism and in early Hashimoto's thyroiditis.
