Diagnostic value of conventional MRI combined with DWI and 1H-MRS in adult medulloblastoma

Abstract:Objective To evaluate the value of conventional MRI combined with DWI and 1H-MRS in diagnosis of adult medulloblastoma.Methods 14 cases with medulloblastoma in adult proved by surgery and pathology were retrospectively analyzed.All cases underwent conventional MRI, 2 cases underwent DWI, 4 cases underwent 1H-MRS.Results Among 14 cases, 5 cases (35.7%) were located in cerebellar hemisphere, 3 cases(21.4%) were located in cerebellar vermis, 2 cases (14.3%) were located in the fourth ventricle, 2 cases (14.3%) were located in cerebellopontine angle (CPA), 2 cases (14.3%) were located in the right cerebellar hemisphere, cerebellar vermis and the right cerebellopontine angle (CPA).2 cases were misdiagnosed, the diagnostic accuracy was 85.7%.In 13 cases (92.9%), cyst and necrosis were seen in the tumor or on the edge.In 4 cases (28.6%), the blood supply vessels were seen in the tumor.For solid component, 9 cases were hypointense and 5 cases were isointense.On T1-weighted imaging, 6 cases were mild hyperintense, 6 cases were isointense, and 2 cases were hypointense.On T2-weighted imaging, 6 cases vere mild hyperintense, 8 cases were isointense on FLAIR.With marked enhancement after Gd-DTPA injection was shown in the solid component of all the cases.The solid components of 2 cases were hyperintense or mild hyperintense on DWI, hypointense or mild hypointense on ADC Map.4 cases 1H-MRS showed the peak of Cho increased markedly, NAA and Cr decreased markedly, Cho/Cr ratio increased, NAA/Cr ratio decreased.Conclusion Conventional MRI combined with DWI and 1H-MRS play an important role in diagnosis and differential diagnosis of adult medulloblatoma.
