MRI imaging findings of acute necrotizing encephalopathy and correlation with clinical studies

Abstract:Objective To evaluate MRI imaging findings of ANE and correlation with clinical pathological studies .Meth‐ods Two cases of ANE with complete clinical data were studied .Their imaging characteristics ,clinical data combined with literature were reviewed .Results Two cases were male ,with typical clinical presentation .The onset of disease was 1 year old and 1 and half year old retrospectively .Both of them had clear history of preceding infection before the onset , lapsed into a coma with convulsive seizure in a short time .The laboratory findings of the CSF revealed the increased level of protein content .MRI showed multiple‐symmetric distribution of lesions ,both involving the bilateral thalamus and brainstem along with hemorrhage ,T1 WI showed hyperintensity ,T2 WI showed mostly hyperintensity ,T2‐FLAIR showed mixed signal intensity .The mild and ring enhancement were observed .The bilateral hippocampus was involved in case 1 . The bilateral white matter of intracerebroventricular ,basal ganglia ,cerebellum were involved in case 2 .Conclusion The most important imaging finding of ANE is the symmetry involvement of the bilateral thalamus ,which often shows hyper‐intensity on T1 WI .
