Clinical application of non-invasive CFR in evaluation of myocardial ischemia caused by coronary double branch lesions of sta -ble coronary heart disease

Abstract:Objective To explore the value of non‐invasive coronary flow reserve (CFR) in evaluating myocardial ische‐mia caused by left anterior descending branch (LAD) and left circumflex (LCX) lesions and to predict the rate of coronary artery stenosis by transthoracic Doppler echocardiography (TTDE) .Methods Fifty‐six patients suffering from chest pain with known or suspected coronary artery disease were performed ATP stress TTDE to measure resting and maximum ex ‐pansion coronary blood flow velocity of LAD and LCX ,then CFR was calculated .All patients were performed coronary angiography (CAG) and single‐photon emission computed tomography (SPECT ) .Results ① To evaluate myocardial is‐chemia ,there was not statistical significant difference between non‐invasive CFR and SPECT myocardial perfusion imaging ( P > 0 .05) .CFR ≤ 2 .0 evaluatiing LAD and LCX lesions corresponding blood regional myocardial ischemia yielded high sensitivity and specificity of 93 .3% & 92 .3% and 84 .6% & 85 .7% ,respectively ; ② CFR ≤ 2 .0 yieldsed a sensitivity of 90 .4% and specificity of 90 .5% to predict coronary stenosis more than 70% significantly ; ③ The success rate of TTDE measuring CFR of LAD was 96 .4% (54/56) and that of LCX was 73 .2% (41/56) .Conclusion As a aided method ,non‐invasive CFR measured by TTDE can evaluate myocardial ischemia caused by LAD and LCX lesion of stable coronary ar ‐tery disease .Using CFR and CAG has important clinical value for choosing treatment of stable coronary artery disease .
