Application of Laryngeal Mask Airway Supreme on 50~60 Kg Female Patients Undergoing Hysteroscopic Operation:Comparison between Size 3 and Size 4

Abstract:Objective To evaluate the application of Supreme laryngeal mask airway(SLMA)between size 3 and size 4 on 50~60 kg female patients undergoing hysteroscopic operation.Methods 120 patients weighing 50~60 kg,aged from 22 to 64 years old with body mass index of 18~28 kg/m2 ,ASA classification Ⅰ orⅡ and Mallampati classification Ⅰ orⅡ were included in this study.Patients all underwent the selective hysteroscopic operation under general anesthesia.They were divided into size 3 SLMA group(group T)and size 4 SLMA group(group F)using a random number table with 60 patients in each group.Patients in group T and group F were inserted size 3 and size 4 SLMA respectively following induction and then received mechanical ventilation.The insertion time,rate of improved insertion technology and successful cases of SLMA placement at first attempt were recorded.The oropharyngeal leak pressure(OLP)was measured after LMA placement.Peak airway pressure(Ppeak),PET CO2 ,fiberoptic bronchoscopy classification,anethesia time,operation time and visible blood stain on SLMA were recorded.Patients were enquired a﹣bout the presence of complications such as sore throat,hoarseness,dysphagia 24-hour postoperatively by an investiga﹣tor who was blinded to the research.Results There were no significant differences in terms of the success rate of SL﹣MA placement at first attempt,Ppeak,PET CO2 ,fiberoptic bronchoscopy classification,anethesia time,operation time and presence of complications of sore throat,hoarseness,dysphagia between the two groups(P>0.05).Compared with group T,the insertion time was longer,the rate of improved insertion technology and visible blood stain on SLMA and OLP were higher in group F(P<0.05).Conclusion Both size 3 and size 4 SLMA can be effectively applied to the airway management of 50~60 kg female patients undergoing hysteroscopic operation,and size 3 is more convenient to insert with lower incidence of mucosal injury than size 4.
