Association between 25-Hydroxvitamin D,Somatic Muscle Weakness and Fall Risk in Elderly Patients with CKD-3 Renal Failure
Abstract:Objective To explore the relationship between the level of 25 -hydroxyvitamin D(25OHD)and in﹣creased fall risk in patients with chronic kidney disease-3(CKD-3).Methods 25 elderly patients with chronic re﹣nal failure(CRF)at CKD-3 stage were included in this cross-sectional study.25OHD,1 ,25 -dihydroxyvitamin D (1,25 (OH)2D)and parathyroid hormone (PTH)were measured.The fall risk was measured by measuring the strength of quadriceps femoris,fall assessment test(FST),Berg balance test(BBS),timed get-up-and-go test (TUG),modified Barthel index(MBI)and fall efficacy score(FES).Results The mean age was (75.8 ±13.2) years,mean disease duration was 5.0 years and the average 25OHD level was 55.6 nmol/L.Muscle strength was sig﹣nificantly positively correlated with 25OHD (P=0.024 )but not with 1 ,25 (OH)2D (P=0.477 )or PTH (P=0.461 ).Conclusion Decreased levels of 25OHD in CKD-3 are associated with reduced quadriceps muscle strength and increased fall risk.25OHD may be more important than active renal metabolite 1 ,25(OH)2D for muscle strength. Further investigation is required to examine effectiveness of calciferol supplementation on the incidence of falls in CKD-3.
