The Correlation between LeuKoaraiosis and Bad Outcomes after Intra-arterial Thrombectomy in Patients with Acute Cerebral Infarction
Abstract:Objective To study the correlation between leukoaraiosis(LA)and bad outcomes after intra-arte—rial thrombectomy in patients with acute cerebral infarction. Methods Clinical data of 49 patients with acute intracra—nial large artery occlusion treated with intra-arterial thrombectomy in Zhongnan Hospital of Wuhan University from Mar. 2017 to Oct. 2018 were analyzed retrospectively. The severity ratings of LA were evaluated by a 4 -point van Swieten scale. Patients were dichotomized into those with moderate-severe LA(2 ~4)versus those with absent-slight LA(0~1). Patients were followed up to get mRS at 3 months. Good outcome was defined as mRS score 0~2 and bad outcome was defined as mRS score 3~6. Hospital deaths(mRS﹦6)were regarded as bad outcome. All pa—tients had the second head CT or MRI scan between 24~36 h after symptoms onset to detect hemorrhagic transforma—tion. The incidence of bad outcomes and hemorrhagic transformation between moderate-severe LA group and absent-slight LA group were compared. Results In all 49 cases,there were 37 cases with absent-slight LA and 12 cases with moderate-severe LA. There were no significant differences on gender,average age,risk factors of cerebrovascu—lar disease,time between onset and visit,responsible vessel and NIHSS scores of the two groups(P〉0. 05). There were no significant differences on hospital mortality and incidence of hemorrhagic transformation between the two groups. Patients with moderate-severe LA had higher incidences(91. 7% VS 59. 5%)of bad outcome than patients with absent-slight LA(P﹦0. 039). Conclusion Severe degree of LA may have no significant influence on hospital mortality and incidence of hemorrhagic transformation,but may increase the incidence of bad outcomes.
