Abstract:Objective Toinvestigateprognosticeffectsofserumalbuminlevelsinadultswithcommunity-acquiredpneumonia(CAP)within24hofadmission.Methods 416CAPpatientsadmittedinthehospitalfrom2013to 2017wereobserved.Clinicalfeaturesandlaboratoryindexeswerecomparedamongdifferentserum albuminlevel groups.Serumalbuminlevelwasindependentvariable.Theprimaryendpointwas30-daymortality.Results The medianvalueofalbuminwas31g/L(IQR28~35).Aslevelsofserum albumindecrease,theriskofcomplications significantlyincreased(P<0.001)withprolongedtimetoreachclinicalstability(P<0.001)andprolongedhospital stay(P<0.001);ICUadmissionrate,theneedformechanicalventilationand30-daymortalitywereallincreased (P<0.001).Afteradjustingforpotentialconfoundersinamultivariatelogisticregressionanalysis,serumalbuminlevels(-5g/Ldecrease)wereindependentlyassociatedwithhigherriskof30-daymortality(OR2.11,95%CI1.73~2.56).Conclusion Serum albuminlevelwithin24hofadmissionisagoodprognosticmarkerinCAP.Physicians shouldconsideralbuminlevelwhenevaluatingtheseverityofillnessforpatientswithCAP.
