Abstract:Objective To investigate the effect of furcation lesions on the prognosis of root canal treatment in primary molars in children. Methods A retrospective cohort study was conducted included children aged 2-8 years who received dental treatment under general anesthesia at the Department of Stomatology,Xi'an Children's Hospital, from 2019 to 2024,with a follow-up period of≥2 years.Clinical and radiographic data were extracted from electronic databases, and exposure factors, outcome variables, and confounding factors were recorded.Patients were divided into Exposure Group 1,Exposure Group 2,and Control Group based on the extent of furcation lesions.Multivariate binary logistic regression analysis was performed. Results At 1 year post-treatment,the risk of failure in teeth with furcation lesions≤1/2 and>1/2 was 3.70 times 95%CI(1.56-9.30),P<0.05 and 9.16 times 95%CI(3.96-23.08),P<0.05 higher, respectively, compared to teeth without furcation lesions. At 2 years post-treatment,the risk of failure in these groups was 2.21 times 95%CI(1.24-4.00),P<0.05 and 6.80 times 95%CI(3.78-12.61),P<0.05 higher, respectively. Conclusion Root bifurcation lesions significantly increase the risk of root canal treatment failure in primary molars, with a higher risk associated with greater extent of lesions.
