A Case Report of Retreatment of a Maxillary First Premolar with Three Root Canals
Abstract:In root canal treatment, the anatomical variation of the root canal system in the maxillary first premolar is relatively common, with most cases having two root canals, followed by single-rooted canals, while the occurrence rate of three-rooted canals is relatively low [3]. Clinically, root canal treatments are mostly focused on the first two anatomical types. However, it cannot be ignored that the anatomical variation of three-rooted canals greatly increases the difficulty of root canal treatment in the maxillary first premolar: first, the exploration of root canals is difficult, and there is a high risk of missing root canals; second, the course and position relationship of the variant root canals are complex, often accompanied by severe curvature and calcification, which increases the difficulty of root canal preparation [4]; in addition, the complicated root canal morphology is not conducive to root canal filling operations. This case report describes the diagnostic process, operational points, and precautions for the treatment of a maxillary first premolar with three root canals, aiming to provide references for the treatment of complex anatomical variations.
