The correlation between serum and gingival crevicular fluid melatonin,HSP70 levels and periodontal indicators and lesion severity in patients with chronic periodontitis

Abstract:AIM:To explore and analyze the correlation between the levels of melatonin and heat shock protein 70(HSP70)in serum and gingival crevicular fluid and periodontal indicators and lesion severity in patients with chronic periodontitis.METHODS:The 120 patients with chronic periodontitis and 120 healthy individuals who underwent physical check ups in Department of Stomatology,Sinopharm North Hospital from May 2021 to May 2024 were selected as the study subjects and included in the periodontitis group and the healthy group,respectively.According to the severity of chronic periodontitis,120 patients with chronic periodontitis were assigned into mild group(n=39),moderate group(n=53),and severe group(n=28).ELISA method was used to detect the levels of melatonin and HSP70 in serum and gingival crevicular fluid.Pearson and Spearman methods were used to analyze the correlation between serum and gingival crevicular fluid melatonin,HSP70 levels with periodontal indicators,lesion severity in patients with chronic periodontitis.ROC curve was used to evaluate the diagnostic value of serum and gingival crevicular fluid melatonin and HSP70 levels in patients with severe chronic periodontitis.RESULTS:The levels of serum and gingival crevicular fluid inflammatory factors IL-6,TNF-α,and HSP70 in the periodontitis group were obviously higher than those in the healthy group,while the levels of IL-10 and melatonin were obviously lower than those in the healthy group(P<0.05).The level of melatonin in the serum and gingival crevicular fluid of the severe group was obviously lower than that of the moderate and mild groups,and the level of HSP70 was obviously higher in the moderate and mild groups(P<0.05).The values of periodontal indicators PLI,AL,SBI,and PD increased sequentially in the mild,moderate,and severe groups(P<0.05).The level of melatonin in serum and gingival crevicular fluid of patients with chronic periodontitis was negatively correlated with periodontal indicators PLI,AL,SBI,PD,and lesion severity,while the level of HSP70 was positively correlated with periodontal indicators PLI,AL,SBI,PD,and lesion severity(P<0.05).The combination of melatonin and HSP70 in serum and gingival crevicular fluid had a higher AUC in the diagnosis of severe chronic periodontitis patients than the single diagnostic value of melatonin and HSP70(P<0.05).CONCLUSION:Melatonin expression is downregulated in serum and gingival crevicular fluid of patients with chronic periodontitis,while HSP70 expression is upregulated.Both are related to periodontal indicators and the lesion severity.
