A Case Report of Microscopic Apical Surgery via Buccal Approach for the Palatal Root of the Left Maxillary First Molar
Abstract:Microscopic apical surgery is an effective method to save teeth that are not suitable for root canal treatment or retreatment. However, microscopic apical surgery on the palatal root of maxillary molars is more difficult and complex due to the distant anatomical position of the palatal root from the buccal side, its close relationship with the maxillary sinus floor, and poor visibility and operability. This case report presents a patient with periapical lesions and maxillary sinus inflammation caused by failed root canal treatment of the left maxillary first molar, which had a fiber post, missed canals, and canal deviation. The microscopic apical surgery of both the buccal and palatal roots was successfully completed via the buccal approach. One and a half years after the operation, the periapical lesion was completely healed, achieving a good therapeutic effect.
