Comparison of clinical outcomes of apical microsurgery: 1-year versus 4-year follow-up

Abstract:AIM:To evaluate the clinical outcomes of apical microsurgery in short-term and long-term follow-up.METHODS:152 patients with 207 teeth underwent apical microsurgery.Clinical and radiographic evaluations were performed 1 y and 4 y after surgery according to Rud's criteria.Based on the follow-up results,the cases were divided into 4 groups,as success-success group,success-failure group,failure-success group and failure-failure group.Cases with different success rates at 1 y and 4 y follow-up were compared,the causes of change and factors of long-term prognosis were analysed.RESULTS:A total of 124 patients with 162 teeth were completed 4 y follow-up (78.3% recall rate).The success rates of 1 y and 4 y follow-up were 88.3% (143/162) and 87.0% (141/162) respectively (P>0.05).6 teeth were classified as success-failure group.Failure reasons at 4 y follow-up were periodontitis,vertical root fracture,secondary caries and traumatic dental injury.4 teeth were classified as failure-success group and the reason of success at 4 y follow-up was interventional treatment after failure reason was found.CONCLUSION:There was no significant difference between the short-term and long-term clinical outcomes after apical microsurgery.1 year outcome can basically predict the long-term prognosis.Strict selection of surgical indications,standardized operation,avoidance of secondary caries and dental trauma can improve the outcomes of apical microsurgery.
