Comparison of the treatment outcomes between different anchorages in vertical control of adults molars with Angle Class Ⅱ hyperdivergent malocclusion

Abstract:AIM:To investigate the difference of mandibular position change as well as soft tissue profile change between conventional anchorage and mini -implants anchorage in vertical control of adult molars with hyperdi-vergent Class II malocclusion.METHODS:32 patients aged 18 -35 with Angle Class II hyperdivergent malocclusion were divided into 2 groups (n =16)and treated with mini -implant anchorage (G1 )and conventional anchorage (G2)respectively.The pretreatment and posttreatment lateral cephalometric radiographs of the patients were used for the measure of skeletal,dental and the profile changes.RESULTS:In sagittal direction,the G1 group showed less mesial movement of the maxillary first molars than G2 group 〔(0.60 ±1.20 )mm Vs (1.63 ±1.04 )mm, (P <0.05)〕.In vertical direction,the maxillary molars were intruded in G1 group by(1.59 ±1.28)mm,whereas they were extruded in G2 group by (0.17 ±0.51)mm(P <0.05).After treatment,G1 group showed a mandibular counterclockwise rotation of (0.57 ±0.45 )°,anterior vertical dimension of the lower face (AFH)reduction of (1.63 ±2.45)mm,and a more prominent chin(vs pretreatment or G2 group,P <0.05).CONCLUSION:Com-pared with conventional anchorage,mini -implants are more effective in the control of vertical maxillary molars for the tooth extraction treatment of adults with Angle Class II hyperdivergent malocclusion.Mandible can be rotated counter-clockwise so that the lateral profile can be more harmonious and beautiful.
