Removal of broken instruments beyond the apical foramen with ultrasonic instruments combined with instrument removal system under dental operation microscope

Abstract:AIM:To evaluate the effect of ultrasonic instruments combined with instrument removal system (IRS)in the removal of broken instruments beyond apical foramen under dental operating microscope.METHODS:28 teeth with broken instruments beyond the apical foramen were included.Among the 28 broken instruments,15 were stainless steel file and 13 were Ni-Ti file.Among the 28 teeth,11 were with curved apical canals and 17 were with non-curved apical canals.4 were anterior teeth and 24 were posterior teeth.The broken instruments were removed by ul-trasonic instruments and IRS under dental operating microscope.RESULTS:16 broken instruments were successfully removed,with a success rate of 57.14%.The success rate of instrument removal was 66.67% (10 /15)in stainless steel file group and 46.15% (6 /13)in Ni-Ti file group.Furthermore,the success rate was 27.27% (3 /11 )in curved apical group and 76.47% (13 /17)in non-curved apical group.In anterior teeth,removal rate was 100%(4 /4)and in posterior teeth it was 50% (12 /24).CONCLUSION:Ultrasonic instruments combined with IRS under dental operating microscope is effective for the removal of broken instruments beyond the apical foramen.
