The predictive value of 24-hour dynamic electrocardiogram combined with Periostin and Gal-3 for malignant ar-rhythmia after acute myocardial infarction

Abstract：Objective To explore the predictive value of 24-hour dynamic electrocardiogram combined with Perios-tin and Galectin-3(Gal-3)for malignant arrhythmia(MA)after acute myocardial infarction(AMI).Methods A total of 367 AMI patients admitted to the Cardiovascular Department of the First Affiliated Hospital of Wannan Medical College(Yijis-han Hospital of Wannan Medical College)from January 2020 to December 2022 were selected as the research objects(AMI group),and 370 healthy volunteers were selected as the healthy control group.Enzyme linked immunosorbent assay(ELISA)was used to detect levels of serum Periostin and Gal-3.All patients underwent 24-hour dynamic electrocardiogram examina-tion and the levels of SDNN,24-hour QTV,SDANN index,and SDNN index were recorded.According to whether MA oc-curred,they were assigned into non-MA subgroup(n=286)and MA subgroup(n=81).The influencing factors of MA in AMI patients were analyzed using Logistic multiple regression analysis.ROC curve was used to analyze the predictive value of Periostin,Gal-3,24-hour QTV,and SDNN for the occurrence of MA in AMI patients.Results Compared with AMI group,the levels of serum Periostin and Gal-3 in healthy control group were increased(t/P=59.225/＜0.001,t/P=33.141/＜0.001).Compared with the non-MA subgroup,levels of serum Periostin,Gal-3,and hypokalemia were higher(t/x2/P=7.602/＜0.001,6.049/＜0.001,15.167/＜0.001).The levels of SDNN,24-hour QTV,SDANN index,and SDNN index in the MA subgroup were lower(t/P=7.029/＜0.001,7.401/＜0.001,4.281/＜0.001,4.721/＜0.001),while the elevated levels of serum Peri-ostin and Gal-3 were risk factors for MA in AMI patients[OR(95％CI)=2.163(1.416-3.303),2.450(1.570-3.823)],while el-evated levels of 24-hour QTV and SDNN were protective factors[OR(95％CI)=0.449(0.264-0.764),0.430(0.223-0.829)].The AUC of 24-hour QTV,SDNN,Periostin,Gal-3 alone and four joint predictions MA in AMI patients was 0.791,0.719,0.758,0.744,and 0.901,respectively.The combined prediction of the four had a higher AUC than single predictions(Z=3.916,6.101,4.888,5.764,all P＜0.001).Conclusion The levels of serum Periostin and Gal-3 are both elevated in AMI pa-tients.The combination of 24-hour dynamic electrocardiogram indicators and serum Periostin and Gal-3 has a higher efficacy in predicting the occurrence of MA in AMI patients.
