Analysis of the short term efficacy of full thoracoscopic assisted mitral valve formation

Abstract:Objective To investigate the short term efficacy and safety of full thoracoscopic assisted mitral valve for-mation. Methods Sixty-three cases of mitral valve disease treated in Xi'an Hi-tech hospital from June 2015 to June 2017 were selected as the research object. The patients were divided into the traditional group (23 cases) and the thoracoscopic group (40 cases) according to the different surgical methods. Traditional open chest mitral valvuloplasty was used in tradition-al group, and VATS assisted mitral valvuloplasty was used in the thoracoscopic group. The operative time, intraoperative blood loss, ventilator assist time, ICU monitoring time, chest drainage volume, hospitalization time, postoperative complications and operation success rate of the two groups were compared. The changes of MVA, Em/Am, E/A, E/Em and PAWP before and after operation were compared between the two groups. Results No death occurred in all two groups,and the success rate was 100% . In thoracoscopic group,intraoperative blood loss,ventilator assist time,ICU monitoring time,chest drainage volume and hospital stay were all less than or shorter than those in traditional group ( t/P=6. 628/0. 000, t/P=4. 719/0. 000, t/P=2. 515/0. 014, t/P=19. 710/0. 000, t/P=4. 385/0. 000). There was no significant difference in the operation time between the two groups (P >0. 05). The MVA in the two groups decreased significantly (t/P =7. 561/0. 000, t/P =10. 067/0. 000), and there was no significant change in Em/Am, E/A, E/Em and PAWP (P>0. 05). Infection, arrhythmia, pul-monary insufficiency in thoracoscopic group was significantly lower than the traditional group (x2/P=4. 432/0. 035, x2/P=4. 143/0. 041, x2/P=6. 273/0. 012). There was no significant difference in the incidence of peri-valve leakage and cardiac insufficiency in the two groups (P>0. 05). Conclusion Full thoracoscopic assisted mitral valvuloplasty is effective and safe for patients with simple mitral valve disease,which can reduce trauma, reduce intraoperative bleeding and chest drainage, shorten respirator assistance time, ICU monitoring and hospitalization time, and reduce postoperative complication rate.
