The relationship between E/E′value and left ventricular remodeling in patients with ST elevation myocardial infarc-tion

Abstract:Objective To evaluate the association between the ratio ( E/E′) of early transmitral flow velocity( E) to early mitral annular velocity(E′) and left ventricular remodeling in ST segment elevation myocardial infarction(STEMI)pa-tients treated with percutaneous transluminal coronary intervention(PCI). Methods From July 2014 to December 2016, 126 STEMI patients treated with PCI in the ICU of the first people' s hospital of shijiazhuang city were selected as the subjects. Two echocardiographic examinations were performed on patients during hospitalization and follow-up. Patients were divided in-to normal LV diastolic function group (E/E′≤15, A group, n =80) and LV diastolic dysfunction group (E/E′ >15, B group, n=46) according to whether the patient developed diastolic dysfunction. The clinical parameters and echocardiograph-ic parameters were compared between the two groups at admission (1. 4 ± 0. 3) days and after surgery (172. 4 ± 29. 1)days to evaluate the correlation of E/E′ on left ventricular remodeling. Results After admission, two groups of patients underwent immediate echocardiography within (1. 4 ± 0. 3) days. Compared with group A, E and E/E′ values were higher in group B (t=5. 701, 12. 029, P<0. 001), and the interval E′ was lower (t=12. 029, P<0. 001). Follow-up echocardiography showed that compared with group A, the left ventricular ejection fraction ( LVEF) in group B was significantly decreased( t=2. 705, P=0. 008), Left ventricular end-diastolic volume index increased significantly (LVEDVI)(t=3. 746, P<0. 001), Left ventricular end-systolic volume index increases (LVESVI)(t=2. 987,P=0. 003),ΔLVEDVI in group B increased sig-nificantly(t=5. 076, P<0. 001) andΔLVESVI increased significantly(t=2. 902, P=0. 004). indicating that the degree of left ventricular remodeling was higher in group B than in group A. The Pearson test showed that there was a linear positive cor-relation between Follow-up E/E′ and LVEDVI,LVESI (r=0. 871, P=0. 023;r=0. 592,P=0. 036),and negatively with LVEF(r= -0. 516,P=0. 047). Conclusion In STEMI patients undergoing emergency PCI, elevated E/E ratio is associat-ed with left ventricular remodeling.
